Your First Visit

Our team will have you fill out our new patient forms either before your visit or when you arrive at our office. We’ll then guide you back to one of our comfortable operatories and capture any necessary X-rays, and Dr. AJ will complete a thorough examination of your mouth in search of any signs of common oral health problems. If anything is spotted, our team will provide you with a treatment plan and answer any questions you may have. Finally, before you leave, our skilled dental hygienist will thoroughly clean, polish, and floss your teeth.
Dental Patient Forms

We ask that all of our new dental patients please fill out their forms online, before their visit. This will help minimize the time you need to spend in our waiting area and provide us with important information beforehand, like your dental insurance information, which will allow us to conduct a benefits check if needed.
